Be absolutely, 100%, certain that it's what you want. Have a level of awareness that it's something that you can't ever undo - like too much salt in something you're cooking, you can always add more, but you can never take it away once it's done.
Get it done safely if you possibly can, involve an experienced cutter if you know anyone who's able to travel to your location to help you out, but in a pinch you can manage it through DIY if you are *very* careful, do your homework, and are willing to shoulder some risk.
Aside from that, don't dawdle. I went dickless when I was just a hair into my 40's, and I wish I'd had it done back when I was 20. With how fundamental the desire was in me, it was part and parcel with my own sense of being, and after finally heailng up from it, I can't even begin to describe how much more self-confidence I have just by virtue of no longer having a dick.
